Job summary

The Nurse Navigator at Mercy coordinates patient care across the care continuum, guiding bedside staff to deliver timely, safe, high-quality care. The role focuses on discharge planning, LOS management, and collaborative care across inpatient and outpatient settings.

Qualifications

• Current RN license in state of practice.
• BLS certification required.
• 2-3 years’ nursing experience preferred.

Responsibilities

• Provide pre-op education to patients and families.
• Conduct daily rounds and monitor care plans.

• Ensure compliance with regulatory guidelines and Mercy Service Standards.
