SSM Health is seeking a Registered Nurse Navigator to coordinate care across the continuum in a clinic setting. You will connect patients with resources and collaborate with multidisciplinary teams to drive positive outcomes and patient satisfaction.

The role requires an RN with at least three years of experience, BLS, and licensure in the applicable states. Day shift with regular hours in an Orthopedic Clinic supports patient-centered care and education.

Qualifikationen

• Graduate of accredited school of nursing or education equivalency for licensing.
• Three years' registered nurse experience.

Aufgaben

• Acts as liaison for management, staff, physicians, managed care, and community partners.
• Collaborates with physicians and APPs to ensure quality outcomes and education.
• Tracks data, outcomes, and recommends process improvements.
• Provides referrals for follow-up care for underinsured patients.
• Applies nursing process (assessment, planning, intervention, evaluation) in patient encounters.
• Assists with processing diagnostic results and clinically relevant information.
• Educates patients and families about medications and therapy.
• Supports patient education, and maintains patient safety.
