Mercy Clinic Orthopedics Southfork is seeking a compassionate RN Nurse Navigator to guide patients through their orthopedic journey from initial consult through surgery, recovery, and follow-up. You will coordinate care with surgeons, therapists, and care teams to ensure timely, high-quality service and an exceptional patient experience.

Responsibilities include educating patients and families, supporting discharge planning, and documenting progress in EPIC.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification required.
• BSN preferred and ACLS certification a plus.

Responsibilities

• Coordinate and navigate care for orthopedic patients across evaluation, treatment, surgery, and recovery.
• Educate patients and families on diagnoses, procedures, plans, and post-operative care.
• Facilitate communication among patients, providers, and care teams for seamless coordination.
• Develop individualized care plans with physicians and interdisciplinary teams.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Maintain accurate documentation in EPIC and other systems; track outcomes and quality metrics.
