Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey, from initial consultation through recovery and follow-up. You will coordinate care with surgeons, APPs, therapists, and interdisciplinary teams to ensure timely, high-quality service.

You will educate patients and families on diagnoses, procedures, and post-operative care, while maintaining clear documentation and contributing to quality

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.
• Preferred BSN in Orthopedic, surgical services, care coordination, or nurse navigator experience.
• ACLS certification.

Responsibilities

• Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families on diagnoses, procedures, treatment plans, and post-operative care.
• Facilitate communication among patients, providers, and care teams to ensure seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
• Participate in quality improvement initiatives and program development.
• Serve as a clinical resource for patients, families, providers, and staff.
