Mercy Clinic Orthopedics Southfork seeks a Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey from initial consultation to surgery, recovery, and follow-up. You will coordinate care with surgeons, therapists, and care teams to ensure timely, high-quality care and a superior patient experience.

In this role, you will educate patients and families, facilitate interdisciplinary communication, and monitor progress while contributing to quality improvement

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsibilities

• Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families regarding diagnoses, procedures, treatment plans, and post-operative care.
• Facilitate communication between patients, providers, and care teams to ensure seamless care coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.

Education

Graduate of an accredited school of nursing
