Job summary

Mercy Clinic Orthopedics Southfork is seeking a Registered Nurse (RN) Nurse Navigator to guide patients through orthopedic care, from initial consultation to recovery and follow-up. You will coordinate care, provide education, and support patients in collaboration with surgeons and multidisciplinary teams.

Qualifications

• RN license in state of practice
• 2-3 years nursing experience
• BLS certification

Responsibilities

• Coordinate and navigate orthopedic patient care from evaluation through recovery.
• Educate patients and families on diagnoses, procedures, and post-operative care.
• Facilitate communication between patients, providers, and care teams for seamless coordination.
• Develop individualized care plans with physicians and interdisciplinary teams.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Document care activities accurately in clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
