Job summary

Mercy Clinic Orthopedics Southfork seeks a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey, from initial consult to surgery, recovery, and follow-up. You will coordinate care with surgeons, therapists, and care teams, providing education, addressing barriers, and ensuring timely, high-quality care and an exceptional patient experience.

This full-time role requires RN licensure, 2–3 years nursing experience, and a commitment to Mercy's

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification required.

Responsibilities

• Coordinate and navigate care for orthopedic patients from evaluation through recovery.
• Educate patients and families on diagnoses, procedures, and post-operative care.
• Facilitate communication between patients, providers, and care teams.
• Develop individualized care plans with physicians and teams.
• Support pre-operative planning, discharge coordination, and follow-up.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to improve patient care.
• Participate in quality improvement initiatives.
• Serve as a clinical resource for patients, families, and staff.
