Job summary

Mercy Clinic Orthopedics Southfork is seeking a Registered Nurse (RN) Nurse Navigator to guide patients through orthopedic care from initial consultation to recovery. You will coordinate care, educate patients and families, and support discharge planning.

Working with surgeons, APPs, therapists, and interdisciplinary teams, you will ensure timely, high-quality care and an exceptional patient experience, with responsibilities including pre-op education, daily rounds, and post-op follow-up.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate and navigate care for orthopedic patients from evaluation through recovery.
• Educate patients and families on diagnoses, procedures, and post-operative care.
• Facilitate communication between patients, providers, and care teams for seamless coordination.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track outcomes and quality metrics to support evidence-based practice.
