Banner Health is seeking an RN Patient Navigator to provide inpatient and outpatient complex care coordination for Advanced Lung Disease patients in Mesa, AZ. This role focuses on safe, appropriate care, patient education, and care plan adherence throughout the continuum.

The Navigator serves as the point of contact for patients and families, coordinating with specialists and primary providers to optimize outcomes in both hospital and clinic settings.

Qualifications

• Bachelor's degree in nursing or related field required.
• Active RN licensure in the state of practice.
• BLS certification required.
• Minimum 5 years clinical experience.

Responsibilities

• Assess patient care plans and coordinate resources to meet health care needs.
• Evaluate medical necessity and arrange initial tests and consults.
• Coordinate care using multidisciplinary teams with physicians and providers.
• Educate patients and families about disease process, treatment, and community resources.
• Support discharge planning and identify barriers to care.
