Qualifications

• Bachelor's degree in nursing and active RN licensure in the state of practice.
• BLS certification and strong clinical knowledge in specialty care services.

Responsibilities

• Assesses and coordinates patient plan of care across the continuum.
• Evaluates medical necessity and coordinates tests/referrals.
• Guides patients and families through diagnostics and care planning; discharges planning.
• Supports end-of-life decision-making and provide emotional support.
