Job summary

LCMC Health seeks an RN Patient Navigator / Lung Nodule Coordinator to serve as a clinical resource, advocate, educator, and care coordinator for patients across the continuum of care, focusing on nodules and complex conditions.

You will maintain a registry, coordinate imaging and referrals, educate patients, and collaborate with interdisciplinary teams to ensure timely follow-up and adherence to guidelines.

Qualifications

• Current RN license in Louisiana in good standing.
• American Heart Association Basic Life Support (BLS) certification.
• Minimum of two (2) years of related nursing experience.

Responsibilities

• Coordinate written and verbal communication regarding the patient's treatment plan among patients, families, physicians, and interdisciplinary care teams.
• Review imaging reports to identify patients with lung nodules and ensure follow-up according to guidelines.
• Maintain a lung nodule registry and track diagnostics, follow-up, outcomes, and adherence to care plans.
• Coordinate diagnostic testing and referrals to pulmonology, thoracic surgery, oncology, radiology, rehab, social work, nutrition, and other services.
• Educate and support patients and families in navigating complex healthcare decisions and treatment-related concerns.
