Qualifications

• Bachelor's degree in nursing required.
• Current RN licensure in state of practice.
• BLS certification required; typically 5 years of clinical experience.

Responsibilities

• Assess the patient’s plan of care and coordinate resources to meet health needs.
• Evaluate medical necessity and coordinate initial tests and consultations.
• Assist patients and families through diagnostics and care with interdisciplinary teams.
• Ensure patients and families understand care plans and provide information and support.
• Establish and implement a plan of care with discharge planning and goals.
• Support end-of-life decision making and process improvements across care transitions.
