The role emphasizes education on oncology treatments, risk factors, and side effects, while coordinating care across physicians, radiologists, and providers to empower patients in decision-making.

Qualifications

• Requires RN license in state of practice.
• Excellent communication and writing skills.
• Strong organizational and time management abilities.
• Experience coordinating care across teams and services.

Responsibilities

• Assess patient plan of care and monitor resource use.
• Evaluate medical necessity and coordinate initial tests and consultations.
• Assist patients and families through diagnostic services and care coordination.
• Explain diagnostic processes, care plans, and actions to patients and families.
• Establish measurable goals and discharge plans with patient/family.
• Support patients and families during difficult decisions and end-of-life care.
• Participate in process improvements to optimize outcomes and satisfaction.
• Collaborate with physicians, payers and health care teams for timely care.
• Operate with independence within established procedures and guidelines.
