Job summary

Responsibilities

• Assess patients' medical, emotional, and socioeconomic needs and coordinate solutions to enhance treatment adherence.
• Provide patient education about treatment and potential side effects to improve understanding and quality of life.
• Facilitate communication between patients, caregivers, and healthcare providers, ensuring timely information sharing.
• Coordinate referrals to social workers, financial counselors, dietitians, and community resources.
