FMOL Health seeks a Patient Navigator to assist patients and families, coordinating referrals and resources across acute and sub-acute care. The role supports discharge planning, readmission prevention, and education, collaborating with physicians and staff to ensure exemplary care.

The navigator will collect data for dashboards, support care transitions, and advocate for patient rights while tailoring education to individual needs.

Qualifications

• 3 years in acute clinical setting with relevant patient populations.
• Bachelor's degree in nursing.
• Proficient in English; strong verbal and written communication; good computer skills.
• Current Louisiana RN license; BLS certification.

Responsabilités

• Clinical practice and care management with patient-specific plans.
• Collaborate with staff to determine treatment and rehabilitation goals.
• Collect specified data and contribute to quality care evaluation.
• Facilitate admission, discharge, and transfer processes.
• Act as a clinical resource for the interdisciplinary team.
