Qualifications

• Virginia RN license required.
• Experience with MDS assessments and RAI processes is preferred.
• Knowledge of PDPM classification is a plus.

Responsibilities

• Schedule and lead interdisciplinary team care plan meetings.
• Implement and interpret MDS assessments.
• Review medical records, nursing documentation, and therapy notes to validate MDS accuracy.
• Code MDS items to reflect resident acuity and PDPM classification.
• Track and manage CMI and Quality Measure data for reimbursement and survey readiness.
