Job summary

Richmond Post Acute in Henrico, VA seeks a skilled Registered Nurse Assessment Coordinator (MDS/RNAC) to lead the MDS assessments and coordinate the RAI process in compliance with federal, state, and facility guidelines.

Responsibilities include scheduling and leading IDT care plan meetings, implementing MDS items, reviewing records for accuracy, coding MDS to reflect resident acuity and PDPM classification, and tracking CMI and Quality Measures for reimbursement and survey readiness.

Qualifications

• Virginia RN license required.
• Experience with MDS/RNAC processes preferred.

Responsibilities

• Schedule and lead IDT care plan meetings.
• Implement and interpret MDS assessments.
• Review medical records and nursing documentation to validate MDS accuracy.
• Code MDS items for accurate resident acuity and PDPM classification.
• Track Case Mix Index (CMI) and Quality Measure data for reimbursement and survey readiness.
