Richmond Post Acute in Henrico, VA is seeking a skilled Registered Nurse Assessment Coordinator (MDS/RNAC) to lead the MDS/RAI process with timely, accurate assessments.

You will schedule IDT care plan meetings, review records, code MDS items for PDPM, and monitor CMI and quality measures to support reimbursement and survey readiness.

Job Summary

The Registered Nurse Assessment Coordinator (RNAC) is responsible for the timely and accurate completion of the Minimum Data Set (MDS) assessments and coordination of the Resident Assessment Instrument (RAI) process in accordance with federal, state, and facility guidelines.

Responsibilities

• Schedule and lead interdisciplinary team (IDT) care plan meetings.

• Implement and interpret MDS (Minimum Data Set) assessments.

• Review medical records, nursing documentation, and therapy notes to validate MDS accuracy.

• Accurately code MDS Items to reflect resident acuity, ensuring appropriate PDPM classification.

• Track and manage Case Mix Index (CMI) and Quality Measure data to support facility reimbursement and survey readiness.
