Job summary
Riverside Health & Rehabilitation is seeking a Registered Nurse Assessment Coordinator (RNAC, MDS) to oversee patient/resident care from admission to discharge. The role reports to the Director of Nursing and requires an active RN license and strong MDS/RAI expertise.

Qualifications
- Must hold a current, unencumbered RN license in state of practice.
- Minimum of 2 years of direct hands‑on nursing experience.
- Minimum of 2 years of long‑term care experience.
- Preferably at least 1 year of RAI experience (focus in MDS).
- Proficiency in ICD 10 coding is required.
- Reliable transportation is required.
Responsibilities
- Identify and monitor resident skilled services.
- Coordinate the Resident Assessment Instrument (RAI) process.
- Complete an accurate MDS and develop the interdisciplinary Plan of Care.
- Monitor patient/resident status and identify changes or barriers to discharge.
- Participate in the daily PPS Interdisciplinary Team Meeting (ITM).
- Work with the nursing management team to maintain relationships and care quality.
