Job summary

Lutheran Senior Services is seeking an MDS Coordinator RN to direct and implement the MDS process and the interdisciplinary Plan of Care for residents. You will join the Nursing Leadership Team, focusing on accurate documentation and resident care.

Responsibilities include CMS/OBRA guided assessments, MDS submissions, quality measures reporting, and collaboration with a multidisciplinary team to ensure high standards of care and regulatory compliance.

Qualifications

• Current RN license in state of practice.
• Graduate of an accredited nursing program; BSN preferred.
• CPR certification.
• Ability to make independent decisions.
• Knowledge of long-term care regulations and CMS/OBRA guidelines.

Responsibilities

• Directs, coordinates, and facilitates the MDS process and development of the Care Plan.
• Maintain MDS status per OBRA, PPS, Medicaid requirements.
• Monitor for changes in resident condition and initiate appropriate MDS/IPA.
• Coordinate with Managed Care to support third‑party reimbursement.
• Support Quality Measures reporting and interdisciplinary collaboration.
• Ensure timely submission of MDS and Care Plans; maintain regulatory knowledge.
