Job summary

Care Initiatives is seeking a compassionate MDS Coordinator (RN) to lead residents’ assessments and care planning within our long-term care communities. You will ensure timely MDS evaluations, collaborate with the care team, and support regulatory compliance.

This role emphasizes accuracy, resident advocacy, and on-going education. You will participate in care conferences, mentor staff, and help maintain survey readiness while prioritizing quality of life for residents.

Qualifications

• Active RN license in good standing.
• Current CPR certification.
• Familiarity with federal and state long-term care regulations.
• Commitment to learning and compassionate care.
• Empathy and discretion when interacting with residents.

Responsibilities

• Complete and coordinate accurate, timely MDS assessments.
• Collaborate with interdisciplinary teams to develop care plans.
• Ensure CMS/state/federal long-term care regulatory compliance.
• Monitor and validate clinical documentation for reimbursement and quality.
• Analyze resident data to support clinical decisions.
• Serve as a resource on MDS processes and documentation standards.
• Participate in care conferences and communicate with residents and families.
• Support audit readiness and maintain survey-ready documentation.
• Enforce policies and guide the team; foster growth.
• Communicate with residents and families about care needs.
• Document care to meet standards.
• Follow infection control and safety protocols.
• Participate in on-call rotation.
