Job summary

Sunrise Senior Living is seeking a Resident Assessment Coordinator (RN/LPN) to lead MDS accuracy and drive quality care outcomes. In this role, you will coordinate MDS timelines, finalize CAA and care plans, and ensure accurate documentation for reimbursement, while collaborating with the care team and IDT meetings.

You will also mentor staff, participate in QAPI activities, and support billing coordination.

Qualifications

• Experience coordinating MDS processes and care plans under federal/state regulations.
• Ability to review and submit clinical documentation for reimbursement and compliance.
• Strong collaboration with IDT, QAPI and billing teams to ensure accurate care planning.

Responsibilities

• Coordinate and complete MDS, CAA and Care Plans per regulations.
• Maintain MDS schedules and ensure timely submissions.
