Job summary

Colonial Village is seeking a dedicated MDS Coordinator - RN to join our nursing leadership team. You will oversee MDS 3.0 submissions and care plan processes, advocating resident-centered care and coordinating with physicians, families, and health professionals.

A registered nurse with MDS experience is preferred, and RAC certification is supported by the company. Responsibilities include coordinating assessments, training staff, and participating in on-call rotations to maintain high-quality

Qualifications

• RN licensed, in good standing, with current CEU requirements.
• MDS experience or willingness to attending MDS training and RAC-certified, paid by the company.
• Strong assessment skills and ability to communicate resident-centered care.

Responsibilities

• Ensure timely completion of MDS 3.0 and CMS submissions.
• Oversee Care Plan initiation and updates.
• Assist in planning delivery of nursing care and resident function goals.
• Support staff training and continuing education in the nursing department.
• Participate in on-call rotation with nurse leadership team members.
