Job summary

Majestic Care of Manchin seeks an experienced MDS Coordinator (RN) to manage MDS and care plans for residents in a timely, compliant manner. The role supports the facility's standards and state/federal guidelines.

Responsibilities include coordinating care plans, PPS/MDS submissions, and team collaboration while upholding resident rights and privacy. CPR certification within 30 days is required. Primary location is West Virginia facility.

Qualifications

• RN or LPN license active in the state of employment.
• Experience in Long Term Care preferred.
• Proficient with MDS 3.0 and data entry/word processing.
• Knowledge of nursing practices, laws, and guidelines for skilled facilities.
• Ability to plan, organize, and implement programs and procedures.
• CPR certification required within 30 days.

Responsibilities

• Coordinate MDS and care plans in accordance with facility policies and procedures.
• Inform care team when a care plan is due and ensure completion by all members.
• Develop and/or revise resident care plans quarterly or with significant changes.
• Manage PPS and state MDS assessments; ensure MDS/CAAS completed.
• Coordinate care plan meetings and encourage team participation.
• Submit MDS to state/federal agencies per Medicare/Medicaid guidelines.
• Maintain resident rights, confidentiality, and privacy in MDS processes.
• Maintain communication across departments as resident conditions change.
• Monitor care during MDS and report concerns to Executive Director and DON.
• Assist with pre-admission screening to estimate potential RUGs as needed.
