Qualifications

• Active RN license required.
• MDS Certification preferred.
• Experience in clinical assessment and MDS coordination.
• Strong knowledge of federal and state regulations related to MDS and care planning.

Responsibilities

• Conduct and coordinate comprehensive assessments of residents using the MDS tool.
• Develop, implement, and review individualized care plans with an interdisciplinary team.
• Ensure compliance with federal and state regulations for MDS assessments and care planning.
• Monitor and transmit MDS assessments to authorities.
• Train nursing staff on MDS procedures and updates.
• Audit clinical records for accuracy and regulatory compliance.
• Collaborate with healthcare providers to optimize resident care outcomes.
