Qualifications

• RN license in Nevada.
• Experience with MDS assessments and care planning.
• MDS license in Nevada preferred.
• RN long-term care experience.

Responsibilities

• Coordinate and manage the MDS assessment process.
• Collaborate with the healthcare team to ensure accurate assessments and timely completion.
• Develop and implement individualized care plans for residents.
• Provide education and support to residents and families to promote independence and quality of life.
• Maintain accurate records of assessments, care plans, and progress.
• Identify and address potential quality of care issues.
