• Collaborate with the healthcare team to ensure accurate assessments and timely completion.
• Develop and implement individualized care plans for residents.
• Provide education and support to residents and families to promote independence and quality of life.
• Maintain accurate records of assessments, care plans, and progress.

As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring that our residents receive the necessary care and services to meet their unique needs. Responsibilities:

• Coordinate and manage the MDS assessment process, including identifying and addressing any issues or concerns

• Provide education and support to both residents and their families to promote independence and quality of life

Requirments:
