Job summary

Phoenix Rehabilitation and Nursing Center is seeking an experienced MDS Assessor to join our team. The candidate will complete the Resident Assessment Instrument and coordinate care as a case manager with physicians, families, and payers.

The role requires an active RN license, at least one year in long-term care, and strong critical thinking. You will lead MDS/CAA, ARD, and RUG scoring and ensure timely electronic submissions and multidisciplinary collaboration.

Qualifications

• Current active RN license in the state of practice.
• Minimum of one year of experience in a long-term care setting.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversee MDS, CAAs and Care Plans per regulations.
• Act as in-house Case Manager coordinating with physicians, families, payers and staff.
• Identify regulatory risks and resource capture for MDS outcomes.
• Capture ARD and RUG score after selecting assessment reason.
• Understand MDS payers: Medicare, Managed Care, Medicaid.
• Ensure timely electronic submission to the state database and follow up on validations.
• Lead Care Management Process with the resident, IDT and family for discharge planning.
• Educate IDT on RAI process and training of new/updated processes.
• Analyze QI/QM data to identify trends monthly.
• Participate in committees as required.
