Job summary

Canton Center for Nursing and Healing, part of Empire Care Centers, is seeking a dedicated MDS Coordinator to ensure accurate resident assessments, high-quality care, and compliance with state and federal guidelines in a collaborative, team-driven environment.

You’ll coordinate all MDS assessments, work with nurses and department heads to ensure timely documentation, analyze QM/QI data for improvement, and uphold RAI/MDS procedures with deadlines.

Qualifications

• Current RN license (Georgia) required.
• Prior MDS/RAI experience in long-term care preferred.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.

Responsibilities

• Coordinate and oversee all MDS assessments in accordance with CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation.
• Prepare and analyze QM/QI data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and submission deadlines.
