Qualifications

• Must hold a current RN license in good standing with the state.
• Experience in hospital, long-term care, or similar healthcare setting required.
• Experience completing MDSs and Care Plans required.
• Knowledge of long-term care laws, regulations, and guidelines.

Responsibilities

• Ensure appropriate reimbursement for patient care services.
• Participate in Patient Assessment Process via MDS and CAAs.
• Develop individualized care plans with the interdisciplinary team.
• Identify patient needs and expected outcomes per regulations.
• Support ongoing staff education and training on RAI/PPs.
