Job summary

Genesis HealthCare, Inc. is seeking a Registered Nurse experienced in MDS and Care Plans to support reimbursement processes and patient assessments.

You will participate in the Patient Assessment Process, complete MDS items, and collaborate with the care team to ensure regulatory compliance and quality outcomes. Ideal candidates have RN licensure in good standing, experience in hospital or long-term care settings, and familiarity with RAI/CAAs.

Qualifications

• Must be a Registered Nurse currently licensed by the state and in good standing.
• Previous experience in a hospital, long-term facility, or other healthcare related facility is required.
• Prior experience completing MDSs and Care Plans is required.
• Knowledge of general, rehabilitative and restorative nursing and medical practices and regulations governing long-term care.

Responsibilities

• Develop individualized patient care plans and identify patient needs and outcomes per Federal/State regulations.
• Contribute to the RAI process and ensure MDS is completed, encoded, and transmitted electronically.
• Identify changes in patient condition and coordinate with the interdisciplinary team for indicated assessments.
• Assist with Activities of Daily Living training for new hires and ongoing education for staff.
• Attend educational programs to maintain knowledge of RAI/Prospective Payment System regulations.
