• RN licensure from an accredited school of nursing, LPNs with MDS experience may be considered.
• Minimum one year of long-term care nursing experience required.
• Ability to coordinate cross-functional care and documentation.”

Responsibilities

• Oversee timely, accurate MDS assessment completion and care plan reviews.
• Manage Medicare/Medicaid case-mix documents to support reimbursement.
• Consolidate information from nursing, dietary, social services, restorative, rehab and physicians.
