Job summary

Monarch Healthcare Management is seeking a Registered Nurse to serve as MDS Coordinator at our Waterview Woods in Eveleth and Waterview Shores in Two Harbors, MN. The role emphasizes accurate MDS completion, case mix maximization, and regulatory compliance, coordinating with the IDT to improve resident care.

Qualifications

• Must be a Registered nurse (RN) in Minnesota.
• CPR certification and emergency care for health care provider preferred.
• Ability to communicate verbally and in writing when directing the RAI process.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to ensure accurate assessments and quality care.
• Ensure regulatory compliance and reimbursement related to MDS processes.
