Job summary

Monarch Healthcare Management seeks an experienced MDS Coordinator (RN) in Minnesota. The role oversees all MDS assessments from editing to transmission, ensuring accuracy, compliance, and timely completion.

You will support the IDT and nursing staff to enhance care plans and resident outcomes. Ideal candidates will have MN RN license and experience with MDS/RAI, with a focus on quality care and regulatory alignment.

Qualifications

Must be a Registered nurse (RN) in Minnesota.
Must have CPR certification or be pursuing CPR/emergency care certification for health care providers.
Must communicate verbally and in writing when directing the RAI process and when caring for residents.
Must be able to perform essential functions with or without reasonable accommodation.
Must qualify for employment after criminal background study, per Minnesota Department of Human Services.
Experience in healthcare, skilled nursing facility or assisted living community preferred.
Experience with MDS and Resident Assessment Instrument (RAI) preferred.

Responsibilities

Oversee completion, editing, locking and transmission of all MDS assessments.
Coordinate care plan completion with the Interdisciplinary Team (IDT) and provide clinical support to nursing staff.
Help assure timely completion of the MDS with focus on accuracy and regulatory compliance.
Support reimbursement processes related to MDS and ensure quality resident care.
