Monarch Healthcare Management is seeking a dedicated MDS Coordinator (RN) to lead MDS assessment completion, editing, locking and transmission, ensuring accuracy and regulatory compliance. The role focuses on maximizing case mix and coordinating care plans with the IDT while supporting nursing staff.

The ideal candidate is a Minnesota-licensed RN with CPR certification and strong communication skills, experienced in healthcare and familiar with MDS/RAI processes.

Qualifications

• Excellent communication skills, both verbal and written.
• Ability to interpret and direct the RAI process clearly.
• Strong attention to detail and regulatory awareness.
• Proven experience in healthcare settings, especially skilled nursing or related facilities.
• Familiarity with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely, accurate MDS completion with focus on case mix maximization.
• Maintain regulatory compliance throughout the MDS process.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to enhance assessments and care plans.
