Job summary

Monarch Healthcare Management is seeking an MDS Coordinator (RN) in Minnesota to oversee the MDS assessment process, ensure accuracy and timely transmission while focusing on case mix and regulatory compliance.

The role includes coordinating care plans with the IDT and providing clinical support to nursing staff to improve assessments and quality of care across Monarch’s facilities. A valid MN RN license and CPR certification are required or pursued.

Qualifications

• Must hold MN RN license.
• CPR certification or willingness to obtain.
• Strong verbal and written communication.
• Ability to coordinate with IDT and care teams.
• Background check required.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Coordinate care plans with the Interdisciplinary Team (IDT) and ensure regulatory compliance.
• Provide clinical support to nursing staff to enhance accurate assessments and quality of care.
