Monarch Healthcare Management seeks an MDS Coordinator RN in Minnesota to oversee the MDS assessment process, ensuring timely and accurate submissions, regulatory compliance, and proper case-mix optimization. You will collaborate with the IDT to develop care plans, support nursing staff, and contribute to quality of care across Monarch's facilities.

A Minnesota RN license and CPR certification are required, with healthcare experience preferred.

Qualifications

• RN licensed in Minnesota
• CPR certification or willingness to obtain
• Ability to communicate effectively in writing and verbally
• Ability to perform duties with or without reasonable accommodation
• Clear background check per Minnesota DHS requirements
• Healthcare setting experience preferred
• MDS/RAI familiarity preferred

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely, accurate MDS completion with proper case-mix maximization and regulatory compliance.
• Coordinate care plan development with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to improve assessment accuracy and quality of care.
