Monarch Healthcare Management is seeking an experienced MDS Coordinator in Minnesota to oversee the MDS assessment lifecycle with focus on accuracy and regulatory compliance. You will support nursing staff and collaborate with the IDT to optimize care plans and reimbursement processes.

The role requires MN RN or LPN licensure, CPR certification, and demonstrated experience with MDS/RAI in skilled nursing or assisted living settings.

Qualifications

• Licensed RN or LPN in Minnesota.
• CPR certification or in progress.
• Experience in healthcare, skilled nursing facility or assisted living.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely completion of the MDS with focus on accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to enhance assessment quality and resident care.
