Job summary

Trilogy Health Services in Indiana seeks an MDS Coordinator (RN) to oversee resident assessments and care planning, ensuring compliance with federal and state regulations related to Medicare/Medicaid reimbursement.

You will lead MDS assessments, CAAs, and care plans, coordinate with care teams, and communicate options to residents and families. This role requires current RN licensure and CPR certification, with several years of MDS or assessment experience in long-term care.

Qualifications

• Must have and maintain a current, valid state RN license.
• Three (3) to five (5) years' experience in an MDS or assessment role in senior residential care, healthcare, senior living, or long-term care (preferred).
• Current, valid CPR certification required.

Responsibilities

• Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including Care Area Assessments (CAA)s and triggers.
• Evaluate each resident's condition and determine any need for special assessment activities or amend the admission assessment.
• Prepare and electronically transmit reports to the national Medicare and Medicaid databases.
• Develop a written plan of care for each resident identifying problems/needs and goals.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs.
• Ensure MDS notes are informative and descriptive of services and resident responses.
• Assist with completing the care plan portion of the discharge plan and implement recommendations from committees.
