Job summary

Trilogy Health Services, LLC is seeking an MDS Coordinator to travel to campuses across Northwest Indiana to cover vacancies and census increases. You will conduct and coordinate resident assessments in accordance with regulatory guidelines, including CAAs and Triggers, and interview residents for comprehensive care planning.

Qualifications

• Nursing degree or state-certified program is required.
• Three to five years in MDS or assessment role preferred.
• Proficient in charting and clinical documentation.

Responsibilities

• Conducts and coordinates MDS assessments per regulations.
• Interviews residents for assessments and updates plans of care.
• Ensures baseline, comprehensive, and periodic care plans are completed timely.
• Communicates requirements to care teams and coordinates CAAs/Triggers.
• Travels to campuses to cover vacancies and increases census.
