Job summary

Trilogy Health Services seeks an MDS Coordinator to support multiple campuses, covering vacancies, leaves and census fluctuations. You will coordinate comprehensive resident assessments, ensure regulatory compliance, and drive timely CAAs and Triggers implementation.

The role requires an RN/LPN license, CPR certification, and 3–5 years in MDS or assessment roles within long-term care environments.

Qualifications

• Nursing degree or state-certified program.
• LPN or RN license required.
• CPR certification.
• 3–5 years MDS or assessment experience.

Responsibilities

• Travels to campuses to cover MDS Coordinator vacancies, leaves, vacations and high census.
• Conducts and coordinates resident assessment (MDS) in accordance with regulations.
• Implements CAAs and Triggers and prepares timely reports for Medicare/Medicaid databases.
• Ensures accuracy of medical records and coding related to resident assessments.
• Develops plans of care and reviews care plans with assessment teams.
