Qualifications

• Three (3) to five (5) years of experience in MDS or assessment role.
• Experience in senior residential care, healthcare, or long-term care environment preferred.
• Effective communication skills and attention to detail.

Responsibilities

• Oversee resident assessment and care planning process.
• Conduct Minimum Data Set (MDS) assessments.
• Prepare and transmit reports to Medicare and Medicaid databases.
• Develop written plans of care for residents.
