Job summary

Trilogy Health Services, LLC is seeking an MDS Coordinator to travel to campuses in Northwest Indiana to cover vacancies and times of increased census. You will coordinate resident assessments (MDS), CAAs, and Triggers, and ensure timely interviews, assessments, and care plans while reporting to national databases.

Requires an LPN/RN license, CPR certification, and 3–5 years' experience in MDS within long-term care; strong Microsoft Office skills are essential.

Qualifications

• Nursing degree from an accredited school or a certification from a State Certified Program.

Responsibilities

• Conducts and coordinates the assessment and evaluation of the resident assessment (MDS) in accordance with regulatory requirements and guidelines that govern the resident assessment, including the implementation of CAAs and Triggers.
• Interviews each resident for the resident's assessment.
• Evaluates resident's condition and pertinent medical data to determine any need for special assessment activities or amendment to the admission assessment.
• Ensures initial resident assessment and baseline/comprehensive care plans are completed timely.
• Ensures quarterly and annual resident assessments and care plan reviews are completed on time.
• Informs assessment team members of requirements for accuracy and completion of the MDS.
• Coordinates with specialists to gather resources for care plan goals as needed.
• Prepares and electronically transmits timely reports to national Medicare/Medicaid databases.
• Ensures accuracy of medical records and coding procedures to reflect services provided.
• Develops written plans of care and revises them as needed.
