Qualifications

• HS diploma or equivalent.
• Minimum 3 years of experience working with individuals with chronic illnesses or disabilities in a service coordination role.
• Current, active, valid and unrestricted RN license in applicable state(s).
• State-specific certification may be required by state law or contract.

Responsibilities

• Identifies members for high risk complications and coordinates care with the health care team.
• Manages members with chronic illnesses to ensure cost-effective and efficient use of benefits.
• Obtains thorough member history to develop individualized care plans.
• Establishes short- and long-term goals with the member and family.
• Identifies members who may benefit from alternative care levels or waiver programs.
• Develops and ensures access to member services through the care plan and authorization processes.
