Job summary

Elevance Health is seeking a LTSS Service Coordinator - RN Clinician in Marion and Hendricks Counties, IN. This field-based role requires travel to client sites with some office attendance, focusing on comprehensive care planning and coordination for members with chronic conditions.

The RN will lead care plan development, coordinate services with non-RN clinicians, and collaborate with medical leadership to optimize member health outcomes.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience with individuals with chronic illnesses in a service coordination or similar role.
• Current, active RN license in applicable state(s) required. Certification may be state-specific.

Responsibilities

• Manage the member's case within licensure scope and develop, monitor and revise care plans to optimize health care across the continuum.
• Provide direction to non-RN clinicians participating in the member's case and coordinate services.
• Interface with Medical Directors/Physician Advisors and Inter-Disciplinary Teams on care management plans.
