Job summary

Elevance Health seeks an LTSS Service Coordinator - RN Clinician to manage members' care in the field within Allen County, IN. The role involves coordinating with non-RN clinicians, developing care plans, and performing telephonic or face-to-face assessments to optimize health outcomes.

The RN will lead care coordination across physical, behavioral, and social services, with travel to client sites as required.

Qualifications

• Minimum 3 years of experience working with individuals with chronic illnesses in a service coordination or case management role.
• Current, active RN license in applicable state(s) required.
• State-specific certification may be required by state law and/or contract.

Responsibilities

• Identify members for high‑risk complications and coordinate care with the health team.
• Manage members with chronic illnesses to ensure cost effective and efficient health benefits usage.
• Obtain thorough member history to develop individualized care plans.
• Establish short and long-term goals with the member, caregivers, and physicians.
• Develop, monitor, and revise care plans to meet member needs.
