Job summary

Elevance Health is seeking an LTSS Service Coordinator – RN Clinician in Eastern Ohio. The role focuses on developing, monitoring, and revising care plans to meet member needs, with emphasis on person-centered care across the continuum.

Field-based travel to client sites is required as part of day-to-day responsibilities. The incumbent coordinates with healthcare teams, conducts assessments, and supports access to services within the benefits structure.

Qualifications

• Must have a high school diploma or GED and at least 3 years of experience in case management or a similar role.
• Active, unrestricted RN license in Ohio is required.
• Bachelor’s degree in health or nursing is preferred for this role.

Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and coordinate member needs.
• Manage chronic illnesses and disabilities to ensure efficient use of benefits.
• Develop and monitor individual care plans with member, family, and providers.
• Collaborate with medical directors and care teams on person-centered care plans.

3+ years experience in case management
