Qualifications

• At least 2 years in health care, including 1 year in care management or LTSS settings.
• RN license active and unrestricted in state of practice.
• Bachelor's degree in a health care related field may be required in some states.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and providers to address needs and goals.
• Monitor care plans, document interventions, and adjust as needed.
• Promote integration of LTSS, behavioral health, and community resources.
• Assess medical necessity and authorize waiver services.
• Prepare for ICT meetings and collaborate with care teams.
• Educate and motivate members using motivational interviewing.
