Job summary

Molina Healthcare, Inc. is seeking a skilled Care Manager RN to support LTSS care coordination in Ohio. You will assess, plan, implement, and monitor member care across disciplines, collaborating with physicians, caregivers, and the care team to optimize outcomes.

Responsibilities include waiver management, service planning, and resource coordination with a focus on quality, cost-effectiveness, and member advocacy. Local travel may be required.

Qualifications

• Active RN license in state of practice.
• Minimum 2 years in health care with care management/coordination experience.
• Experience with disabilities, chronic conditions, LTSS preferred.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members and care network.
• Monitor effectiveness and adjust interventions as needed.
• Facilitate inter-disciplinary team meetings for service approval.
• Use motivational interviewing to support member change.
• Travel 25–40% may be required depending on state/contractual requirements.
