Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support LTSS-focused care coordination across multiple counties in Ohio.

You will assess member needs, develop and monitor care plans, coordinate waiver services, and collaborate with a multidisciplinary team to improve outcomes while controlling costs. Responsibilities include conducting home visits, facilitating waivers enrollment, and engaging with members, caregivers, and providers.

Qualifications

• At least 2 years in health care with 1 year in care management or LTSS.
• RN license active and unrestricted in state of practice.
• Some states require a health-related bachelor's degree depending on contract.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently with minimal supervision.

Responsibilities

• Complete comprehensive member assessments within regulated timelines and home visits as needed.
• Facilitate waiver enrollment/disenrollment and coordinate service plans with the care team.
• Develop, implement, and monitor care plans to address member needs and goals.
• Promote service integration across behavioral health and LTSS with community resources.
• Monitor effectiveness of care plans, document interventions, and adjust as needed.
• Identify barriers to care and address psycho/social, financial, and medical obstacles.
